Care Support (CCS) Referral Process
Thank you for considering to use Community
Care Support. In order to start using our services, you will need
to fill out our initial contact referral form.This form is avaliable
online here. You can also fill
the template based form found below. Please fax or email the form
using our contact details found here.
This form will ask you basic questions
about your self and highlight the areas you need help with.We
will then arrange to meet you to discuss your needs and expectations.
We will carry out an assessment of your needs and agree with you
how we can meet those needs.Please note,we are not a domiciliary
care agency and do not provide personal care.We are currently
seeking registration with CQC in order to provide this service
in the near future.
A care plan is then written up showing
the required interventions to meet your needs.A care plan is a
detailed account of duties which the support worker needs to do
when they visit you. We will then allocate a regular worker or
a set of workers depending on your needs.We will introduce you
to other workers in case your regular worker or workers are unavalible
due to various reasons such as annual leave and sickness.
Our workers do not wear uniforms when
in the community as we aim to provide a discreet service. All
our workers are aware of infection control measures and will wear
aprons and protective when needed. All workers will carry an ID.
CCS aims to provide an exceptional
service with a high degree of continuity and commitment at all
times. We are committed to provide highly trained and experienced
staff at all times.Your service will be reviewed periodically
to ensure we are still meeting your needs. However, Care Workers
will monitor your care plan and if at any time it is felt your
needs have changed a review will be held. You can also ask for
your care plan to be reviewed at any time.
CCS aims to provide support as agreed
in your care plan in such a manner that enables you to feel confident,
supported, reassured and satisfied. We aim to involve you and
your family (with consent) about any aspect of your care.
CCS promises to always listen to you, be open and honest with
you and work towards continual improvement of our services. We
aim to be sensitive to the potential loss of dignity and privacy
associated with having people visiting your home.
CCS aims to ensure that every person being cared for is given
respect as a unique individual, regardless of status, gender,
religion, age, race, culture, language and their ability to contribute
to the community.
